For Care Managers: Referring a Family for Support Brokerage

Care Managers are often the first to know a family needs a Support Broker. Here's how to refer a family to us, and how we'll work with you.

Where we are

Spectrum Arch is a Clifton Park nonprofit in the process of offering Support Brokerage, and we are completing OPWDD's Support Broker training and authorization. We're not serving families yet, but we're building a waitlist so families have a broker lined up when we can begin.

Who we expect to serve

  • People eligible for OPWDD services and enrolled in the HCBS Waiver, or close to it
  • Families who want to self-direct with Budget Authority, including those already self-directing and looking for a new broker
  • Families in Saratoga, Albany, Schenectady, Rensselaer, and the surrounding counties
  • Help in English, Urdu, Hindi, Punjabi, and Pashto

How to refer a family

  1. Get the family's permission to share their contact details with us.
  2. Use our waitlist form: choose "A Care Manager or other professional", and put the family's preferred name and contact details in the note. Or email info@spectrumarch.org.
  3. Keep it minimal. Please don't send diagnoses, Medicaid numbers, or records by email. We'll arrange a secure way to exchange anything needed later.

How we'll work with you

  • We'll confirm we received the referral.
  • When we're able to begin, we'll contact the family directly and keep you informed with their consent.
  • We'll coordinate with you on the Life Plan and work alongside the family's chosen Fiscal Intermediary.
